埤頭四武宮位在彰化縣埤頭鄉和豐村埔尾路51號[1]，其主祀溫、康、趙、馬－四府元帥，同祀的還有李、池、吳、朱、范-五府千歲，及二位曾府千歲和金府千歲、玄天上帝、殷元帥、中壇太子元帥、福德正神，大殿龍邊配祀韓夫人媽和值年太歲星君，大殿虎邊配祀五營將軍[2]，另外右廂房一樓為地藏殿，二樓為觀音殿。關於四武宮的整體建築、剪粘、浮雕、壁畫讓信眾覺得都非常吸引，有種舒適親切感，如此傲人的成績，都是禮聘鹿港老師傅彩繪裝飾的傑作。
