灝府千歲早期由位於西溪灣仔內以茅草竹籬蓋成的廟宇供祀。日治時期因恐神像遭搗毀，由灣仔內爐下十二戶弟子輪流於各戶間藏拜。
中華民國接管臺灣後，與二府千歲、吉府千歲合祀於西溪中港以石頭建成的廟宇。該廟前有一中港溪，中港橋樑常被風雨摧殘斷橋，致西溪村民無法擔牲禮過橋至中港犒軍敬茶禮拜，故將三王爺吉府千歲迎回西溪建賜安宮，大王爺灝府千歲、二王爺二府千歲之正身奉於灣仔內西賢宮，中港福安宮得王印重新雕塑大王爺、二王爺而供奉之。
