此佛通常為天人形象，身紅色，國土為「西方無量功德藏世界」[2]（或作上方無量功德藏淨土）。此佛憐憫眾生，尤其發願幫助短命枉死兇折者得到長壽。若有短命者，願意至诚皈依此佛，並念诵此佛之号一百零八遍，以此得以延壽[3]，或者听到此佛名号能一心誠敬，虔誠向此佛祈禱，也能增加寿命[4]。
此佛通常持一寶瓶，內裝不老不死甘露、有時上面還插有楊柳枝，意思是為人治病、延壽，故又稱作甘露王佛。其真言為《聖無量壽決定光明王神咒》，藏傳佛教稱之為《長壽佛咒》（又有《長壽佛心咒》），漢傳佛教中《長壽佛咒》被列入《十小咒》之中，主要意義在透過聖無量壽決定光明王如來功德，祈求延壽，獲得諸天善神擁護，死後往生「西方無量功德藏世界」。
在西藏，其脅侍為尊勝佛母、白度母，號稱「長壽三尊」。在漢地，其脅侍有延命普賢、延命地藏；或者上方日光菩薩、上方月光菩薩等。
